Why Home-Based Care Matters
In India and across the world, the cost of healthcare continues to rise, making hospitalisation and outpatient services increasingly inaccessible for many families. Home-Based Care (HBC) offers a practical solution that not only reduces the burden on hospitals but also ensures that patients receive the dignity and comfort of being cared for in their own homes.
Patients tend to recover better in familiar surroundings, and caregivers feel more empowered when care is community-based. HBC helps bridge the gap between medical institutions and everyday life, fostering trust and continuous care.
How It Works
The HBC model is built on collaboration between a Base Hospital or Clinic and a trained field team. Each HBC team consists of:

A Family Physician, operating from the base hospital.

A Family Health Assistant (FHA), providing day-to-day home care.

A Healthcare Professional, such as a trained nurse, who manages equipment, diagnostics, and transportation.
Training and Capacity Building
Our HBC program includes a 6-month intensive training for nurses and FHAs, conducted in a hospital setting. Trainees learn clinical and community-based skills, equipping them to provide ethical, high-quality care under a physician’s supervision.
The training emphasizes:

Bedside care and basic clinical procedures

Communication and record-keeping

Using mobile-based diagnostics like portable lab kits (for sugar, cholesterol, malaria, dengue, etc.)

Mobile ECGs and telemedicine tools (e.g., WhatsApp consultations) to link field teams with doctors in real time.
Partner With Us
We invite hospitals, clinics, NGOs, and healthcare educators to collaborate with us to start or strengthen Home-Based Care programs in their regions. Our team provides complete training, mentoring, and implementation support to replicate this model effectively.
